Other Registration Info and Forms

Player movements to SCMHA:
Players moving to SCMHA from other Associations are considered new players
and therefore must complete registration in paper form (see new players registration tab)
and provide a copy of the player's birth certificate. 
In addition, they must have a PCAHA player movement Form 133 completed. 
Please see link below for this form.  Please note - there are a number of supporting
documents required to accompany the player movement application. 
Players moving to SCMHA from another association within PCAHA must have the previous
association sign Form 133 as well.  Everything must then be sent to PCAHA for approval.

Player Movements out of SCMHA to any other PCAHA Association will require the above form
to be completed and signed by the President, Vice President or Registrar.  All other information
and forms should be provided by the new Association.

Overage Exemptions
Players requesting to play one division lower because of small size, ability or both must complete
an overage exemption form.  This option is only allowed in 'C' hockey and the player will be
assessed to determine if the movement request is valid.
