New / Returning Players - Paper Registration
2017-2018 Registration
New and Returning Players must complete the forms below in order to properly register with the
Pacific Coast Amateur Hockey Association.
FOR NEW PLAYERS ONLY - In addition to the forms below, a copy of the player's birth certificate
must also be provided at the time of registration.
All forms, birth certificate and registration $$ must be received and processed by the Registrar before
the child will be permitted to participate on ice.

Medical Information - New players will receive an email from EPact to sign in and submit the required medical information. Returning players, please ensure your information on EPact is up to date. An email will be sent to you to confirm your information.
